BACKGROUND:The Client is seeking psychological services from the Psychologist to address specific concerns or issues. The Psychologist is a licensed and qualified professional providing psychological services in accordance with the laws and regulations of the relevant jurisdiction.

TERMS AND CONDITIONS:

 

  1. Purpose of Services: The Psychologist agrees to provide psychological services to the Client for the purpose of addressing the Client's concerns, improving mental well-being, and achieving therapeutic goals.

  2. Confidentiality: The Psychologist will maintain the confidentiality of all information shared by the Client during sessions (as per the law § 100 zákona cÌŒ. 108/2006 Sb., o sociálních službách a zákona § cÌŒ. 101/2000 Sb. O ochranÄ› osobních údajů), except as required by law or when necessary to protect the Client or others from harm. Ethical issues related to confidentiality may arise if the Client's information is requested by a court or if the Psychologist believes there is a risk of harm to the Client or others.

  3. Boundaries and Dual Relationships: The Psychologist will maintain professional boundaries and avoid dual relationships with the Client that may compromise the therapeutic relationship. Ethical issues related to boundaries and dual relationships may arise if personal or non-professional relationships interfere with the therapeutic process.

  4. Informed Consent: The Client acknowledges that they have been provided with information about the nature and purpose of psychological services, including potential risks and benefits. The Client consents to participate in therapy and agrees to raise any questions or concerns about the treatment process.

  5. Fees and Payment: The Client agrees to pay the fees for services as outlined in the Psychologist's fee schedule. Ethical issues related to fees and payment may arise if financial arrangements become a source of conflict or exploitation.

  6. Cancelation policy: The client must notify the psychologist about the cancelation of the appointment at least 24 hours prior to the appointment. In the situation when the psychologist is not notified in advance - the client agrees to pay the full cost of the session. 

  7. Termination: The Psychologist reserves the right to terminate the therapeutic relationship for reasons such as non-payment, failure to make progress, or any other ethical concerns that may arise during therapy. The Client also has the right to terminate services at any time with appropriate notice.

  8. Record Keeping: The Psychologist will maintain records of the Client's treatment for 5 years, which will be kept confidential in accordance with legal and ethical standards. The Client has the right to request access to their records, with exceptions as required by law.

  9. Professional Ethics: The Psychologist agrees to adhere to the ethical standards and guidelines established by their licensing board and professional associations. The Client is encouraged to raise any ethical concerns they may have about the Psychologist's conduct.

  10. Emergency Procedures: The Client understands that the Psychologist may not be available 24/7 and agrees to follow emergency procedures if they are in crisis. Ethical issues related to emergency procedures may arise if the Client's safety is not adequately addressed.

  11. Dispute Resolution: In the event of a dispute arising from this Agreement or the therapeutic relationship, the parties agree to first attempt resolution through mediation or another alternative dispute resolution method.

  12. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which the Psychologist practices.

  13. Entire Agreement: This Agreement represents the entire understanding between the parties and supersedes any prior agreements or understandings, whether oral or written.
